Week 7 DNP Portfolio Assignment Guide

Purpose

The DNP Portfolio is a collection of scholarly work and writing that depicts the development of your knowledge, competency, and achievement of the AACN DNP Essentials (2006). The DNP Portfolio provides you with an opportunity to highlight the knowledge and competencies learned in the DNP program and the completion of the DNP practice change project for their successful transition to the role of DNP-prepared nurse and healthcare systems leader.

Instructions

For this assignment, you will submit all files in a single ZIP file. Each individual file should be labeled with your last name and DNP Portfolio  (For example:  Soprano. DNP Portfolio – Personal Philosophy Paper). The DNP Portfolio should include the following components:

    1. Provide your project abstract. The abstract should be a brief (150-250 words) overview of the DNP practice change project to include the following:

    1. Develop a statement on your commitment as a DNP-prepared nurse leader to diversity, equity, and inclusion in healthcare. Describe how you will promote and advocate for the following:
      1. Sense of belonging among healthcare team
      2. Dignity for all patient populations
      3. Justice for individuals with limited or no access to healthcare services

DNP Practice Change Project Abstract

Background: The project aimed to evaluate the impact of technology with motivational interviewing on medication adherence among inpatient Schizophrenia patients.

Problem: The project addressed medication adherence among Schizophrenia patients.

Methods: The project adopted a pre-test and post-test analysis approach. Fifty consecutive Schizophrenia patients following up with treatment participated in the project.

Intervention: This DNP project pursued a nurse-led intervention to improve medication adherence and symptom management.

Results: The questionnaire questions were put into four categories; Specific-Necessity, Specific-Concerns, General-Overuse, and General-harm.The Specific-Necessity had five items with scores ranging from 5-25. The mean score for the five items before the intervention was 18.2, and the mean score after the intervention was 19.2. The change in score (95% CI) was 1.0 with a p value of 0.005, yielding significance. The Specific-Concerns had 6 items, with scores ranging from 6-30. The mean score of the items before the intervention was 15.3, and the mean score after the intervention was 13.8. The change in score (95% CI) was -1.5, with a p value of 0.002, yielding significance. The General-Overuse category had 4 items, with scores ranging from 4-20. The mean score of the items was 11.4 before the intervention and 11.8 after the intervention. The change in score (95% CI) was 0.4, with a p value of 0.177. The General-Harm category had 4 items, with scores ranging from 4-20. The mean score for these items was 10.1 before the intervention and 9.9 after the intervention. The change in score (95% CI) was -0.2, with a p value of 0.624. No significant changes were observed in general overuse and general harm scores.

Conclusions: Motivational interviewing has a positive impact on medication adherence.

Sustainability of the Project: Leadership support and adequate financial and human resources are critical to the sustainability of this project.

Personal Philosophy on Contribution of DNP-Prepared Nurses to Healthcare Outcomes

My personal philosophy asserts that DNP-prepared nurses have the right knowledge, skills, and competencies to influence healthcare outcomes. The DNP program provides the highest level of evidence-based practice knowledge that would allow DNP-prepared nurses to adopt evidence-based approaches to improve healthcare outcomes (Baylor University, 2023). DNP-prepared nurses are also decisive leaders able to navigate the complex healthcare system and make difficult decisions and critical judgements to improve patient outcomes and translate research evidence into nursing practice (Beeber et al., 2019). DNP-prepared nurses are well-positioned to address current and future needs to improve care access, health equity, and other issues the healthcare system faces. Additionally, nurses are an integral part of the interprofessional approach towards improving the health of the nation.

I have an in-depth understanding of key concepts and processes in organizational systems, research translation into practice, implementation science, and nursing research. This knowledge will allow me to navigate the healthcare system, identify areas of improvement, potential interventions, and adopt evidence-based approaches to help improve healthcare. The leadership knowledge and the understanding of organizational systems will allow me to influence decision-making in healthcare reforms, offering valuable evidence-based insights into policies, practices, and strategies to improve the productivity, efficiency, and cost-effectiveness of healthcare organizations (Edwards et al., 2019). I can identify future trends in healthcare and nursing practice and help my organization adapt to the changes in the healthcare system to remain relevant and competitive. I can also pass the knowledge gained in the DNP program to new nurses and nursing students as an educator and help create curricula and evaluate programs to ensure the support of effective nursing education for a better future of nursing practice and patient care.

I am committed to promoting a diverse, equitable, and inclusive healthcare workforce that is responsive to the diverse patient population, helping provide equitable and inclusive care. I will promote ethical practice, ensuring that patients are treated with the utmost respect and inherent human dignity, and empathetic, compassionate, and culturally sensitive care that prioritizes patients’ needs and preferences and respects patients’ values, cultural and religious beliefs, and worldviews. I will influence policy formulation at the institutional and legislative level to increase access to healthcare services for vulnerable populations, including people with low income, older adults, people with special needs, immigrants, and youth. I will promote the expansion of health insurance coverage to the vulnerable population to increase access to care.
